Maent yn amlinellu\u2019r cymorth sydd ei angen arnynt mewn addysg, iechyd, a gofal ac yn gosod nodau i\u2019w helpu i baratoi ar gyfer bod yn oedolyn. Mae\u2019r nodau hyn yn canolbwyntio ar bedwar maes allweddol: <\/span><b>Cyflogaeth, Byw&#039;n Annibynnol, Cynhwysiant Cymunedol, ac Iechyd.<\/b><\/p>\n<p><span style=\"font-weight: 400;\">Bob blwyddyn, cynhelir adolygiad i wirio cynnydd. Mae\u2019n helpu pobl ifanc i gasglu tystiolaeth o\u2019u cyflawniadau drwy gydol y flwyddyn, y gallant ei rhannu yn eu hadolygiad EHCP.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Er enghraifft, gallai person ifanc sy&#039;n gweithio ar deithio&#039;n annibynnol anelu at gynllunio a mynd ar daith bws.
